Membership Cards and Photographs:
Past members:
- Cards - You can use last year's card again, simply register for an instant renewal if there are no changes (see boxes above), or complete the membership form if there are revisions since 2007. New and replacement cards cost $5 each. Indicate the name of the person whose card is being replaced. No new picture is needed.
- Photos - If the picture on your card does not accurately represent you, e-mail us a current photograph (.jpg format only) with the membership form and payment. Photos must be sent to oppr@opkansas.org. Include your name and phone number along with the .jpg photo. Replacement cards will cost $5 each.
- Proof of residency - You must show us proof of your Overland Park address to establish residency: a photocopy of your driver's license, utility bill, voter registration card or a voided check with a current address on it. If you register by the Internet, you can email a digital file (.jpg format only) of proof of residency to oppr@opkansas.org. You also can fax a photocopy to 913/895-6365. If you register by mail, you can include the photo with your registration form and payment.
- Initial ID cards - Free.
- Photos - Photos need to be current headshots at least 2 inches x 1.5 inches. Indicate each person's name with the photo. You can e-mail photos to oppr@opkansas.org. If you are mailing the registration form, put the mailing address on the back of each photo (for identification and so it can be returned). Family photos won't be accepted. Non-usable photos will be returned and another requested.
If you don't have a photo, you can still mail in your registration form and payment. Just e-mail, send in or walk-in your photograph at a later date.
When all the steps have been properly completed, your card will be mailed to you.
Membership cards are designed to be used for several years at OP's community centers, pools and other activities sponsored by the Parks and Recreation Department.
