Establishments
A license is required by any person, firm, partnership, association or corporation who wishes to operate an in-office massage therapy establishment or massage establishment.

The application may be obtained below or from the City Clerk's office. The annual fee for the establishment is $300.

Therapists
If you want to perform or provide massage therapy or in-office massage therapy, you need to file the following application with the City Clerk and pay an annual filing fee of $50.
